        <title id="abstract-title-109be1ff307944df88e0ab30df330e78">Abstract</title>
        <p id="paragraph-5eec891529294ae6b19e40a5a420ef3c">The term “P-drug” refers to the preferred or personal drug of choice for a particular clinician for a specific medical condition. The P-drug concept enables better medication prescriptions and enhances treatment adherence.<bold id="strong-6f4c00198c4443d7b680cf9b91870de2"> </bold>The aim of the study is to assess the knowledge, attitude and perception regarding ‘P-drug concept’ among medical interns, Postgraduates and Physicians to prescribe drugs rationally.<bold id="strong-e8767a33517d48639b4108c8641f1aae"> </bold>The study is a prospective, cross-sectional pre-validated questionnaire-based study conducted in the Gulbarga Institute of Medical Sciences, Kalaburagi. A total of 150 participants includes Medical interns (50), Postgraduates (50) and Physicians (50) from GIMS were enrolled and instructed to fill the questionnaire using Google form. Majority of the participants know the meaning and main purpose of P drug concept. Physicians and postgraduates having better knowledge of P drug concept compared to Interns. Positive attitude observed among health care professionals. All participants accepted that P-Drug Concept should be taught in detail to medical professionals. Poor perception observed among Interns and Postgraduates. <bold id="strong-2419da8e96954cd49e32ff4bd75ee845">Conclusion:</bold><bold id="strong-8d7bfcfa5c3345cf828bf37fe2369f58"> </bold>The P-drug concept is essential tool to improve the quality of health care, improve the knowledge of essential medicine and practicing rational use of medicine. This study highlighting the necessity for educational intervention attitude enhancing awareness of P-drug concept among medical interns and postgraduates.</p>

      <p id="paragraph-84e84f64434749d5ae3671ea9ad16ab3">In the field of pharmacology, the term “P-drug” refers to the preferred or personal drug of choice for a particular clinician for a specific medical condition. <sup id="superscript-ea82e6ef46bf4eeca0d9b1c55b8eb1ce"><xref id="xref-f995ec8edccc4951bcc6bce3d4e2a9e2" rid="R278413433793167" ref-type="bibr">1</xref></sup> The P-drug concept encompasses the selection of the specific medication, dosage form, dosage schedule, and duration of treatment for a particular patient. This concept is a crucial aspect of rational drug prescribing, enabling clinicians to make informed and effective decisions regarding medication selection for their patients. <sup id="superscript-6ebd2ff268c4470fb7aba29798c734ad"><xref id="xref-1ae8a86fc64c472288d4cd4e27003504" rid="R278413433793180" ref-type="bibr">2</xref></sup></p>
      <p id="paragraph-1fdd8fdcc1f544d79773419c6ea159db"> The WHO defines rational use of medicines as “Patients receive medications appropriate to their clinical needs, in doses that meet their requirements, for an adequate period of time, and at the lowest cost to them and their community. <sup id="superscript-c2de97c489ff4b7f94a5c7be4fa03e0f"><xref id="xref-058688ac28f746ff8fa43434c4a0a0b0" rid="R278413433793172" ref-type="bibr">3</xref></sup> The World Health Organization (WHO) reported that 50% of patients do not take their medications as recommended and that 50% of all medications are prescribed, distributed, or marketed inappropriately. Prescribing is always a challenging task which requires knowledge of essential medicine, rational use of medicine and personal drugs. <sup id="superscript-e981367e7a894b529e4973f3eddb35e7"><xref id="xref-e3a22583dc6241f1bb4601b943060659" rid="R278413433793170" ref-type="bibr">4</xref></sup></p>
      <p id="paragraph-05720d828d85477e838da038357c39c3">The development of a P-drug list involves a comprehensive evaluation of various medications available for a specific therapeutic indication. Clinicians consider factors such as efficacy, safety, cost, and convenience when selecting their P-drugs. Convenience factors include the dosage form, administration route and dosing frequency which influence patient compliance and adherence to the treatment regimen. <sup id="superscript-a21adc29f66c4e2fa5af5daba3f4e368"><xref id="xref-f849a795c85b4977b68b9a2d62a13a95" rid="R278413433793182" ref-type="bibr">5</xref></sup></p>
      <p id="paragraph-f8e5470468964868899532e9dd1c1847">The application of STEP criteria aids the treating physician in creating a logical treatment strategy for each unique patient. <sup id="superscript-d15d22ca4f0a4154818199a99fe7cc7c"><xref id="xref-38e0bfc3b226498ebf57c15d9ed87227" rid="R278413433793183" ref-type="bibr">6</xref></sup></p>
      <p id="paragraph-68e48555949c4addbaf73227173bd9b6">The following are the six steps criteria: </p>

        </list-item>

          <p>Identify the goals of therapy </p>
        </list-item>
        <list-item id="li-0a3243818141">
          <p>List the treatment options (indication oriented) </p>
        </list-item>
        <list-item id="li-f100affb9fdc">
          <p>Provide the rationale for the best treatment for this patient </p>
        </list-item>
        <list-item id="li-d02c3d75e191">
          <p>Write a definitive therapy plan (prescription) </p>
        </list-item>
        <list-item id="li-91570aa811b3">
          <p>Determine the monitoring parameters/follow-up </p>
        </list-item>
      </list>
      <p id="paragraph-bcfba35cf96345809f0c942215a3065b">The P-drug idea promotes a more logical approach to medication prescription by encouraging evidence-based practice and critical thinking among healthcare practitioners. Clinicians can help patients get better results, pay less for healthcare and feel more satisfied by implementing the P-drug approach. Studying health care professional’s knowledge, attitude and perception (KAP) is a useful way to gauge their comprehension, convictions and actions on a given subject.<sup id="superscript-959202e0bff24e99b8bb7bedeee109af"> </sup><xref id="xref-733b6d297464423aa06a4569758f705f" rid="R278413433793173" ref-type="bibr">7</xref> This research study has been undertaken in order to analyse the knowledge, attitude and perception regarding ‘P-drug concept’ among health care professionals to increase the patient safety and drug efficacy.</p>

        <p id="paragraph-cfbbaf4b7470494c97b06e53cbda06c6">A questionnaire designed using Google forms. The questionnaire consisted of three sections which included questions on Knowledge (10), Attitude (10) &amp; Perception (5).<sup id="superscript-d370682e038040e0a5fd8d8022b72441"> </sup><xref id="xref-fcae23ad2f3d43b1ae5d2c779fc25801" rid="R278413433793179" ref-type="bibr">8</xref> Each question consisted to multiple options out of which the participants had to choose single most appropriate option.</p>
      </sec>
      <sec>
        <title id="t-1bb2631f397d">
          <bold id="strong-ff0ee6b760d347fd9f7e47c71a9c40df">Statistical Analysis</bold>
        </title>
        <p id="paragraph-fa1d1dee6b3c41348b09d3765ed0c572">Data was entered into Microsoft Excel spread sheet and data was analysed using descriptive statistics and results were expressed in numbers  and percentage. Chi-square test was used to compare categorical data. <sup id="superscript-c1ca3d261ed740ed94ca67c88aa69248"><xref id="xref-cbd5c271f311405ba94437d927a0090a" rid="R278413433793174" ref-type="bibr">9</xref></sup></p>

      <sec>
        <title id="t-c72325e1ea2e">
          <bold id="strong-22d43a0d8f7f4457849d9f0deaae9236">Ethical considerations</bold>
        </title>
        <p id="paragraph-745edffe737344efa8afdb3429890825">Ethical approval <bold id="strong-e561b94808c94657bdf6442dc90cdf70">(No.GIMS</bold><bold id="strong-d933c3b864fd480399483d60ebcdcebc">/KLB/PHARMA/IEC/342/202 5-26) </bold>was obtained from the institutional ethical committee of the medical college. Participants were informed of the purpose of the study and their right to withdraw at any time. <sup id="superscript-c9f379bb6ca9420f8373cb0b9174b9e0"><xref id="xref-fb24bcb63d394882a951e87fbb49f124" rid="R278413433793169" ref-type="bibr">10</xref></sup> Confidentiality of participants’ information was maintained throughout the study. <sup id="superscript-3c897a95702947689b8100ec8eb24eb8"><xref id="xref-9fbf19ad16a04f29aa2089c8c400f996" rid="R278413433793175" ref-type="bibr">11</xref></sup></p>

      <p id="paragraph-47442fff77474af08a67dfdaae5b78e9">A total of 150 participants includes Medical interns (50), Postgraduates (50) and Physicians (50) from GIMS were enrolled and instructed to fill the questionnaire using Google form.</p>

      <p id="paragraph-7a914abfb58f4b1e85ed6ea8123945a5">Majority of the participants know the meaning and main purpose of P drug concept. Majority of the participants know the difference between a P-Drug and a P-Treatment.<bold id="strong-ba482bb1ec2849398be81930f074df6d"> </bold>Physicians (94%) and Postgraduates (68%) having better knowledge of P drug concept compared to Interns (38%).<bold id="strong-4eba75e5d1084e19b08719ad80272d20"> </bold>This suggests that the P-drug concept is fairly well-known among Physicians and Post graduate students.</p>
      <p id="p-56442862c962"><xref id="x-180e5b8d6e41" rid="table-wrap-5094f55dd4234c6e9cb04dd2f8dea9f6" ref-type="table">Table 2</xref> s<bold id="strong-5f5f8c30111c41fd8355035669d22bcc"/>hows there is a statistically significant association between profession group and knowledge level.</p>
      <p id="p-bad064017aea">The majority of Physicians (98%), Postgraduate students (96%) and interns (82%) have a positive attitude towards the P-drug concept. All participants accepted that P-Drug Concept should be taught in detail to medical professionals. Majority of the health care professionals aware of the common adverse effects of the drugs that commonly prescribed by them. But majority of the interns not aware of the advantages of using P-drugs list. </p>
      <p id="p-8692b619d824"><xref id="x-d946b38e72d4" rid="table-wrap-0817331a439a4c63b77c1bd14cfeaeb0" ref-type="table">Table 4</xref> s<bold id="strong-b021ecef2e8443b8bdf68910999cfcc8"/>hows there is a statistically significant difference in Attitute score between Interns, Postgraduates and Physicians. </p>
      <p id="p-f6005069c5d0">Majority of the Physicians (94%) having good perception towards P-drug concept. Poor perception observed among Interns (04%) and Postgraduate students (10%). Majority of the participants agreed to encourage their colleagues in promoting rational use of medicines in their practice. Majority of the participants felt that teaching programme is needed creating awareness for selection of P drug list among practitioners.</p>

      <title id="title-da788e857ba444cc8a032a850ba5e2e0">DISCUSSION</title>
      <p id="paragraph-7ff055c441f24d6eb379dd47bb772980">A P – drug should be selected based on logical, deductive process including comprehensive and objective information. It is a part of good prescribing skills that promote the rational use of medicine and enhances knowledge about essential medicines. <sup id="superscript-175922cf70804c2bb438d56c746bc184"><xref id="xref-a6a06a9edfd744e48bf4112fb6398c51" rid="R278413433793168" ref-type="bibr">12</xref></sup> P-drug practice is a worthy and constructive concept that should depend only upon prescriber’s professional skill and not on the advice of senior doctors or copying reputed clinician or inducement from sales representatives. In promoting the rational use of medicine, knowledge on P-drug is very important as it is one of the contributors toward rationalization. <sup id="superscript-113315186def4d398ebb0de9402aa6dd"><xref id="xref-569b6c7802ee44eab38e1e1274552779" rid="R278413433793177" ref-type="bibr">13</xref></sup></p>
      <p id="paragraph-a41cd418ca3a4166ae1ac7830539dbd2">The P – drug concept is crucial for the prescribers to choose the best drugs with maximum benefit &amp; minimum side effects. Health care professionals can help patients get better results, pay less for healthcare and feel more satisfied by implementing the P-drug approach. Studying health care professional’s knowledge, attitude and perception (KAP) is a useful way to gauge their comprehension, convictions and actions on a given subject. This research study analyse the knowledge, attitude and perception regarding ‘P-drug concept’ among Medical Interns, Post graduate students and Physicians to increase the patient safety and drug efficacy.</p>
      <p id="paragraph-5abdbfaef6d54f64b5c948eebc4c69e6">In this study majority of the participants know the meaning and main purpose of P drug concept. Majority of the participants know the difference between a P-Drug and a P-Treatment.<bold id="strong-32010b953dda4ed5b336fe7b2f3ea5cc"> </bold> Physicians (94%) and Postgraduates (68%) having better knowledge of P drug concept compared to Interns (38%).<bold id="strong-2bd664d7523d490583fbb5e3925fc1df"> </bold>This suggests that the P-drug concept is fairly well-known among Physicians and Post graduate students. </p>
      <p id="paragraph-f48e1b04ebd74f6b87996f482c135330">The present study we observed that among post graduate students, 88% participants are aware of P-drug, 68% are aware of P-treatment, 98% participants are including fixed-dose combinations in their P-drug list and 58% participants are aware of the advantages of prescribing P-drug. These findings compared with similar study done by Rao SKT et al found that 49.7% participants are aware of P-drug, 43.4% are aware of P-treatment, 28.4% are not including fixed-dose combinations in their P-drug list and 42% are aware of the advantages of prescribing P-drug. <sup id="superscript-4fa9a7574a314fec84ca329f51c783ff"><xref id="xref-5af5736703f24cf9b980c061dd650979" rid="R278413433793179" ref-type="bibr">8</xref></sup></p>
      <p id="paragraph-3f55fc6c095c422dbaa9e53aaae443ed">In this study, the majority of Physicians (98%), Postgraduate students (96%) and Interns (82%) have a positive attitude towards the P-drug concept. All participants (100%) accepted that P-Drug Concept should be taught in detail to medical professionals and 68% of the participants aware of the advantages of using P-drugs list. These findings compared with similar study done by Jyothi DB et al observed that 97.3% felt that teaching programs were needed for preparing P-drug list and 33.6% were aware of advantages of prescribing P-drug. <sup id="superscript-ecf73a75271a480a8a4863e114bfabbb"><xref id="xref-d48cab9c8c41443b85670d034c20451b" rid="R278413433793181" ref-type="bibr">14</xref></sup></p>
      <p id="paragraph-93a2687de7ac4d5189df7333f201bfe9">Majority of the health care professionals (96%) aware of the common adverse effects of the drugs that commonly prescribed by them compared with a study done by Swetha K et al found that 88% participants aware of the common adverse effects of the drugs prescribed by them. <sup id="superscript-6fda077bad36457d89e1122205339a8a"><xref id="xref-d8a9b7f976f0450a8784e2ca59168e19" rid="R278413433793184" ref-type="bibr">15</xref></sup></p>
      <p id="paragraph-5865ee596b2b45c883bd6bdf82d8a529">The present study we observed that majority of the Physicians (94%) having good perception towards P-drug concept. Poor perception observed among Interns (04%) and Postgraduate students (10%). 98% participants felt that teaching programmes are necessary to create awareness and practice of P drugs, and 66% participants did not attend any CME/teaching programmes about the P-drug list. These findings compared with a study done by Swetha K et al observed that 87 % participants felt that teaching programmes are necessary to create awareness and practice of P drugs, and 85.2 % participants did not attend any CME/teaching programmes about the P-drug list. <sup id="superscript-2da330b89ffc40ac8756b1b84ce42057"><xref id="xref-38335414d0f2410b819cb18996b88622" rid="R278413433793184" ref-type="bibr">15</xref></sup></p>
      <p id="paragraph-7293c0ead8b840eda166955d51dc0147">Early implementation of P-drug exercises in undergraduate medical training should be introduced because present discrepancies demonstrate poor familiarity and confidence toward prescribing among undergraduate students. <sup id="superscript-6d09ef976d054a3eabbde60cb9e42b1e"><xref id="xref-a444a352deae4434abcb8aa4764f0f7d" rid="R278413433793178" ref-type="bibr">16</xref></sup> The National Medical Commission (NMC) of India has integrated the concept of Personal Drugs (P-drugs) into its Competency-Based Medical Education (CBME) curriculum. <sup id="superscript-64df8a32821f444f8ea902e5bc0a73ad"><xref id="xref-3bb20e0ba1064c6ca9dfd8f9214d8848" rid="R278413433793176" ref-type="bibr">17</xref></sup> This integration will enhance rational prescribing, reduce medication errors and standardize treatment approaches among future medical graduates. This will lead to improved patient safety, better pharmacological decision-making, and a long-term positive impact on public health.</p>
      <sec>
        <title id="t-1256dab3fb24">
          <bold id="strong-a5ed1fb2da364f21a620faa09386ef1a">Recommendations</bold>
        </title>
        <list list-type="bullet">
          <list-item id="li-dc4b0a83e189">
            <p>Further education and enhanced training include workshops, seminars and CMEs on P-drug concept should be provided to Intern and Postgraduate students. </p>
          </list-item>
          <list-item id="li-1e28c4fc68b9">
            <p>Educational materials like hand-outs, posters and online resources on the P-drug concept should be made available to medical professionals. </p>
          </list-item>
          <list-item id="li-ca1ac01af6f9">
            <p>Research should be conducted to identify most effective educational interventions for promoting the use of the P drugs. </p>
          </list-item>

      <sec>
        <title id="t-2cf2d7c2446f">
          <bold id="s-7d5c2c1d8c83">Limitations</bold>
        </title>
        <p id="paragraph-9a795f52eb7f4bd3865dccd3fe43bd21">The study results reflect self-reported knowledge, attitudes, and practices, which may introduce response bias. The study was conducted in a single medical institution and with a smaller sample size, limiting the generalizability of the findings. The study primarily evaluates theoretical knowledge and self- reported practices rather than actual prescribing behaviours, which would be a stronger indicator of competency.</p>
      </sec>
    </sec>
    <sec>
      <title id="title-f0ad2f8c023a4726992fc5c64ee85cb3">CONCLUSION</title>
      <p id="paragraph-69161e7bfc6c48028ec0fafdb2c31342">In order to provide rational treatment and prevent economic burden to the patient, WHO has recommended ‘Guide to Good Prescribing’, a guidance for selecting and prescribing the best suitable drug for an individual patient, i.e., P-drug concept. <sup id="superscript-6a2ce38c5bae400d97de09f714ddccaf"><xref id="xref-442d7844f1264b7f91023eaa68ca13e1" rid="R278413433793171" ref-type="bibr">18</xref></sup> The present study we assessed the knowledge, attitude and perception regarding ‘P-drug concept’ among Medical Interns, Post graduate students and physicians. We observed that majority of the participants know the meaning and main purpose of P drug concept. Physicians and postgraduates having better knowledge of P drug concept compared to Interns. Positive attitude observed among health care professionals. All participants accepted that P-Drug Concept should be taught in detail to medical professionals. Poor perception observed among Interns and Postgraduates. This study highlighting the necessity for educational interventions aimed at enhancing awareness of P-drug concept among medical professionals.</p>
